Our CME Commitment

This Continuing Medical Education Calendar is
designed to give you accurate information
regarding all category 1 credit CME programs
sponsored by Forum Health Trumbull Memorial
Hospital (TMH) and Forum Health Western
Reserve Care System (WRCS). It is our intention
to provide quality CME programs directed to our
regional physicians. All physicians are invited

to attend.

Topics for presentation are determined by
committees, needs assessment, and quality
assurance issues. We welcome any input you may
have for future CME programs. If you have a
special need, and/or wish to be placed on our
permanent mailing list, please contact

And Forum Health - Western Reserve Care System
are accredited by the Ohio State Medical Association
to provide continuing medical education activities
for physicians.

Designation Statement

Forum Health Trumbull Memorial Hospital or
Forum Health - Western Reserve Care System
designates these educational activities for
category 1 credit (for the number of credits
indicated for each activity) toward the AMA
Physicians Recognition Award. Each physician
should claim only those credits that he or she
actually spent in the educational activity.
