After scheduling an appointment, a risk analysis questionnaire will be sent to your home to complete. During your visit, your personal risk will be assessed and individual counseling, self-exam instruction, education and recommendations will be reviewed.
You, your doctor and SIGNALS staff will work together to ensure that you receive the maximum benefits of the program. Your physician will receive a copy of your risk analysis report to provide follow-up care.
